Getting into the Salter Point Aged Care Facility is a breeze! With our straightforward process, transitioning into our care is easy and hassle-free. Remember, the process may vary slightly depending on your individual circumstances, but these are the general steps you’ll need:

1. CONTACT US

2. VISIT THE FACILITY

Schedule a visit to the Salter Point Aged Care Facility.

During the visit, you can check out the facilities, meet the staff, and ask any questions you may have.

 

3. APPLY FOR ADMISSION

You can apply for admission during your visit to the facility. We will provide you with an application form to fill out.

4. ARRANGE AN ASSESSMENT

You’ll need to arrange an assessment with the Aged Care Assessment Team (ACAT). This assessment helps determine your care needs and eligibility for aged care services. You can contact an ACAT directly, or your doctor or local hospital can provide a referral to your local ACAT.

5. FINANCIAL ASSESSMENT

You may need to undergo a financial assessment to determine your contribution towards the cost of care. This assessment is usually conducted by the Department of Human Services or the Department of Veterans’ Affairs.

6. APPLICATION APPROVAL

After your application is processed, we will reach out to you or the person you’ve nominated to inform you whether a suitable room matching your specific care requirements and financial circumstances has become accessible

7. MOVE IN

Once a place becomes available, you can move into the facility. The staff will assist you with the transition and ensure you’re comfortable in your new home.
